Even if we’ve never had a negative experience, some of us have to brace ourselves just to make a call to a dentist’s office. And it’s not because we’re worried about a cavity, gum disease, or another diagnosis. It’s the thought of the cleaning. The metal scraping on the teeth. The cold water hitting that sensitive spot on the upper right. And the time it takes for the cleaning. Yes, we know how important it is to schedule regular cleanings, but there’s got to be a better way.
And there is.
It’s called Guided Biofilm Therapy or GBT for short. And at Van Scoyoc Periodontics & Implants in Southern Pines and Sanford, NC, we’re excited to offer this new, advanced approach, giving our patients access to a cleaner, healthier smile without the discomfort.
What Exactly Is GBT?
GBT is a new method created by Electro Medical Systems in Switzerland to remove dental biofilm—the bacterial film that forms around your teeth and along your gumline every day. The Swiss technology is based on four decades of clinical research to replace old-fashioned dental cleanings with one that’s comfortable, painless, effective, and quick. The result? Reduced risk of tooth decay, infection, and disease. And happier, healthier smiles.
How GBT Works
Biofilm is just the clinical term for plaque, the starting point of nearly every oral health issue, from cavities to gum disease. While traditional cleanings tackle it well—and have done so for generations—GBT takes a different approach.
Step 1: Disclose
The first step is to see what’s happening on your teeth. A “disclosing solution” is applied to your teeth. If you grew up chewing those little purple or pink dye tablets at the dentist’s (or even in elementary school)—you know, the ones that lit up every little spot you missed while brushing—this will feel familiar.
The solution stains biofilm (i.e., plaque) a visible color. This lets your clinician map out exactly where the buildup is hiding, so they know where to focus their cleaning. There’s no guesswork. Nothing gets missed.
And just as it was for children, it can also be genuinely eye-opening for patients as they discover any gaps in their brushing routines. So, you can improve your dental hygiene at home, knowing exactly where you need to focus.
Step 2: Airflow
Most of us have become accustomed to sitting back as a hygienist diligently scrapes hardened biofilm off the teeth. With GBT, the process is markedly different. Instead of using a scraper, a wand delivers a gentle jet stream of warm water and fine erythritol powder to blast away the stained biofilm and soft deposits. The stream is both precise and controlled. Best of all, the water is warm (not cold), so it’s dramatically more comfortable—especially for those of us who have sensitive teeth.
This step is revolutionary for people who have found past cleanings uncomfortable or even painful. Many describe it as surprisingly relaxing—like a spa session for our mouths.
Step 3: Perioflow
For patients who need a deeper clean due to deeper gum pockets or dental implants, a specialized tip can be used to reach below the gumline. That means areas that are unreachable with a toothbrush or floss can be addressed gently and easily. This is one of the biggest benefits of GBT.
Step 4: Piezon
If any hardened calculus (i.e., tartar) is found after the Airflow step, a gentle ultrasonic scaler is used to remove it. Yet because the biofilm has already been cleared, this step is used only where it’s truly needed. This minimizes unnecessary contact with healthy teeth and soft structures, which both saves time and promotes comfort.
Step 5: Check & Prevent
The final step is to confirm that all biofilm has been removed and your smile is clean and fresh. We can then walk you through a personalized home-care plan that addresses where you may have been missing when cleaning your teeth. Rather than providing generic advice (“Don’t forget to brush and floss!”), we can provide targeted tips for your mouth.
Why GBT Is So Game-Changing
Many of us have sensitive teeth and gums, which can make it hard to make those vital cleaning appointments. If you can relate, GBT was designed with you in mind. The warm water eliminates the cold-water shock that causes so much discomfort. The fine powder is safe for tooth enamel, exposed root surfaces, and soft tissue alike. And because metal instruments are used minimally (and only where necessary), the entire process is easier and more effective. You may wonder why no one came up with this method sooner!
Beyond this improved comfort, other advantages of GBT include:
Less time in the chair (hurray!). Because the disclosing step maps out the biofilm precisely, your clinician can work efficiently and purposefully.
That also means nothing gets missed. The dye doesn’t lie. Every pocket of biofilm is visible before treatment begins.
Finally, GBT is safe for restorations. If you have veneers, crowns, or implants, the powder-and-water method won’t damage those surfaces.
Who Is GBT Best for?
Honestly, it’s a good solution for just about anyone. However, it’s even more valuable for certain patients, such as:
- People with sensitive teeth or dental anxiety. If past cleanings have been painful, stressful, or unpleasant, the GBT experience is genuinely different. It may even help rebuild trust.
- People with implants, which require gentle, non-abrasive cleaning to protect the surface. GBT has been found to be one of the safest, most effective options available.
- Patients with periodontal pockets as the Perioflow tip can access the deeper areas affected by periodontitis. The deeper cleaning—below the gumline—is more effective than conventional methods.
- Teenagers and young adults, especially those who tend to rush through brushing. The disclosing step shows exactly which areas aren’t getting enough attention, and this lesson can be brought home for more effective cleaning.
- Orthodontic patients, as braces, wires, and brackets are notorious for trapping biofilm in hard-to-reach areas. The Airflow step easily gets around the hardware to ensure the teeth are cleaned thoroughly.
